In an effort to improve the quality of care, safety, satisfaction and outcomes of heart failure patients while driving cost performance improvements, Sentara Virginia Beach General Hospital created a Heart Failure Interactive Care Plan to actively engage patients during every step of their care, from admission to discharge and beyond. By leveraging interactive bedside technology from GetWellNetwork, Sentara was able to ensure that all patients are educated about the disease, understand their role in maintaining a healthy lifestyle and are motivated to take more responsibility for their own health. Within the first year of implementing the plan, Sentara experienced significant outcomes improvements.
