Boston hospitals cut medication errors
By integrating their computer systems and improving their processes, Brigham & Women's Hospital and Massachusetts General Hospital of Boston were able to cut down on medication errors during the med reconciliation process. The two reduced the error rate from a standard 1.44 errors with potential for harm to 1.05 during the course of their study. Article
