NQF releases care coordination, patient safety measures

In a raft of new measures, the National Quality Forum on Friday endorsed 12 care coordination measures, including patient medication reconciliation, advanced care planning and the timely availability of medical records.

Among other newly endorsed measures, the standards-setting organization touted 22 quality measures on cancer care, two patient safety measures on venous thromboembolism prophylaxis and current medication documentation in medical records, as well as 12 disparities and cultural competency measures to ensure patient engagement across ethnicities.
