California state investigators found that more than 7,300 patients at a San Diego-area hospital may have been exposed to infections caused by contaminated drugs last year, according to Kaiser Health News. Investigators found that the compounding pharmacology lab at Paradise Valley Hospital in National City had dust, stains and foreign material in the supposedly sterile environment where thousands of medications were prepared. The inspectors also found that the hospital's head of infection control failed to monitor the lab and also that she forged documents to cover-up her failure to check operating room practices in 2015.
