State inspectors have found dozens of violations of health and safety codes at the University of Iowa Hospitals, including incidents related to the deaths of nine patients. State inspectors found the violations when visiting the hospital on behalf of CMS. The inspection was sparked by an incident this summer when a patient wandered away from the facility and was found 800 miles away at a relative's house.

Violations cited in the state's report included the deaths of nine restrained patients, dangerous problems with the hospital's kidney dialysis treatment program, outdated medications and medical supplies and inadequate infection control procedures. The hospital has since said that it has addressed the problems and educated staff members.
