With the Centers for Medicare & Medicaid Services penalizing 30-day readmissions, it's more important than ever for hospitals to take a proactive approach to prevent readmissions. It seemed like an uphill struggle, considering two-thirds of hospitals received fines in the second round of penalties last year. Recently, however, there are signs that hospitals made headway in this ongoing battle. Not only have Medicare readmissions fallen for the second consecutive year, individual hospitals and systems nationwide found innovative ways to improve population health and reduce readmissions.
