The Indiana State Medical Association has slapped Anthem Blue Cross and Blue Shield with a complaint alleging that the health plan isn't following the rules set by a 2005 settlement with the AMA and medical societies. It says the plan is violating claims processing and payment practice requirements. Release [1]
