15 communities chosen for ONC-funded data exchange program

AcademyHealth, a health services research program, has chosen 15 communities that will take part in its Community Health Peer (CHP) Learning Program.

The CHP Program is being funded by the Office of the National Coordinator for Health IT, which is providing $2.2 million to AcademyHealth to identify best practices for population health management.

Each of the communities AcademyHealth chose will receive $100,000 for the 17-month process of creating plans to improve community health through information exchange and use of electronic health data. Announcement
